Course Summary

Indicative Module Content: Factors which are unique to long term conditions as opposed to acute illness; the impact of long term conditions; the assessment and management of common long term illnesses such as asthma, obesity and metabolic syndrome, Type 1 DM, hypertension, angina, COAD, osteoporosis, osteoarthritis, inflammatory bowel disease, eczema and other skin disorders; self-managed care; motivation and empowerment of patients and families; new approaches to delivering care eg case management and Expert Patient Programme; strategies for representing the needs of patient populations.

Intended Learning Outcomes

At the end of the module you  will be able to:

  1. Critically discuss the assessment of patients with long term conditions and their potential differential diagnoses
  2. Critically evaluate appropriate investigations which will assist in the diagnosis and ongoing management of long term conditions
  3. Critically review the management of long term conditions in line with best evidence showing understanding of the impact of psycho-social factors on patients living with these conditions;
  4. Critically reflect on the involvement of the patient and family in patient care and issues therein;
  5. Critically reflect upon their role as it concerns working with populations of patients with long term conditions.
